Request medical records

Parents and legal guardians

Parents and legal guardians who want to request their child’s medical records can:​

• Submit a signed authorization form via fax or bring it to your MD Family Clinic office. Healthcare providers can also request that your child’s records be sent directly to their office with a completed authorization form.​

• Use our ROI vendor, VitalChart, for full medical charts or additional documentation.​ Most requests are processed within 5–10 business days, though older records may take longer.

Third-party requestors

• All third-party requestors must submit an Authorization for Release of Protected Health Information (available in English and Spanish). For patients under age 18, this form must include a parent's or legal representative's signature. In some cases, additional documents may be required.

• Third-party requestors can use VitalChart to fulfill their medical record request.

Basic records


Basic records such as immunization records, school/camp forms, lab results, and last office visit notes may be available for pickup directly at your MD Family Clinic office. These records can also be made available through the patient portal.

Entire chart


Requests for additional records or a full medical chart must go through VitalChart. For fee estimates and details, please contact VitalChart. Fees and processing time vary depending on the request type and delivery method (digital vs. mailed).
